Claude Code のメモリファイルを Obsidian Vault に自動同期するプラグイン。Write ツール使用時とセッション終了時に ~/.claude/projects/*/memory/*.md を、環境変数 OBSIDIAN_VAULT_NAME（推奨）または OBSIDIAN_VAULT_PATH（フォールバック）で解決された Vault パス配下の Claude-Memory/ へ同期する。
