Injects a full effort doctrine at SessionStart (read from skills/effort/SKILL.md) and re-asserts a cheap trigger every turn via UserPromptSubmit. Universal — applies to every task. Toggle with /effort off | on. No Stop hook, no post-turn gate; governance is in-generation, the way caveman governs terseness.
